In the form, you assign an Agent and specify financial responsibilities to your Agent; for example, you may only want your Agent to be able to make stock trades for you. In the form you can choose to have the Delaware Financial POA go immediately into effect, or you can choose to have it only go into effect when you become incapacitated and unable to communicate (this is called a "springing" Power of Attorney). You can terminate a Power of Attorney at any time, and any Power of Attorney becomes invalid at your death. Health Care POAThe Delaware Health Care Power of Attorney Form allows you to create a custom Health Care Power of Attorney (often referred to as a Medical POA). In the Health Care POA Form you appoint an "Agent" to make health care decisions on your behalf. You have the right to create a Health Care POA under Chapter 16, Section 2501, et seq., of the Delaware Code. This Delaware Power of Attorney only goes into effect in the case you become incapacitated and are unable to make your own health care decisions.
