Use the Nevada Power of Attorney Form to build your own customized Power of Attorney (POA). A LegacyWriter form is a sophisticated web questionnaire that allows you to complete your document online securely. After completing the questionnaire you will have the option to buy your document. Following purchase, download and make changes at any any time to your document for 30 days for free. LegacyWriter provides POAs for Financial and Health Care situations.
The Nevada Financial Power of Attorney form allows you to appoint an "attorney-in-fact" to act upon your behalf regarding financial matters. An attorney-in-fact is not an attorney, but is instead your "Agent" who makes decisions in your place. In a Financial Power of Attorney you can specify which matters of finance you want your Agent to be able to handle; for example, you may only want your agent to be able to make Real Estate transactions for you. In the form you can choose to have the Nevada Financial POA go immediately into effect, or you can choose to have it only go into effect when you become incapacitated and unable to communicate (this is called a "springing" Power of Attorney).
Health Care POA
The Nevada Health Care Power of Attorney Form allows you to create a custom Health Care POA (often referred to as a Medical POA). In a Health Care POA you appoint an "Agent" to make health care decisions on your behalf. You have the right to create a Health Care POA under Sections 449.800, et seq., of the Nevada Code also known as the Durable Power of Attorney for Health Care Act. This Nevada Power of Attorney only goes into effect in the case you become incapacitated and are unable to make your own health care decisions. If you have a Living Will your Agent will have the ability to interpret and enforce your wishes in the Living Will. With LegacyWriter you can create a Health Care Power of Attorney and a Living Will at the same time, by beginning either questionnaire.
