The Ohio Power of Attorney Form allows you to make your own Power of Attorney (POA) document. With LegacyWriter the form is a secure web questionnaire that asks questions specific to the state of Ohio. As you progress though the questionnaire helpful information will be displayed that enables you to create your own legal and valid document. When you finish the questionnaire, decide whether to purchase your document. Following purchase, you can make changes and download your document as often as you want for one month. LegacyWriter provides optional printing and shipping services for an additional fee. POAs for Financial and Health Care situations are available.
The Ohio Financial Power of Attorney Form gives you the ability to make your own custom Financial Power of Attorney online. In the form, you assign an Agent and specify financial responsibilities to your Agent; for example, you may only want your Agent to be able to make stock trades for you. In the form you can choose to have the Ohio Financial POA go immediately into effect, or you can choose to have it only go into effect when you become incapacitated and unable to communicate (this is called a "springing" Power of Attorney). You can terminate a Power of Attorney at any time, and any Power of Attorney becomes invalid at your death.
Health Care POA
The Ohio Health Care Power of Attorney Form allows you to create a custom Health Care Power of Attorney (often referred to as a Medical POA). In the Health Care POA Form you appoint an "Agent" to make health care decisions on your behalf. You have the right to create a Health Care POA under Section 1337.11, et seq., of the Ohio Code, also known as the Durable Power of Attorney for Health Care Act. This Ohio Power of Attorney only goes into effect in the case you become incapacitated and are unable to make your own health care decisions. If you also have a Ohio Living Will your Agent will have the ability to interpret and enforce your wishes in the Living Will. With LegacyWriter you can create a Living Will and a Health Care Power of Attorney at the same time, by beginning either questionnaire.
